Grace M. Czerniakowski, age 78, of Sylvania, passed away on April 12, 2016.  She was born on March 5, 1938 to Anthony and Dorothy Smarzynski in Toledo, OH.  Grace was a graduate of Notre Dame Academy, earned her nursing degree from Mercy School of Nursing and her associated degree in gerontology from Lourdes College.   She was a dedicated nurse at the Toledo Hospital where she worked over 30 years.   Grace was an avid reader,  enjoyed cooking and baking cookies.   She cherished the time she spent with her family and enjoyed going to her children and grandchildren’s activities and sport events.  
Grace was preceded in death by her parents.   She will be dearly missed by her loving husband of 51 years, Florian ‘Red’; children, Mike (Maureen), Tony, Annie and Gary (Jill); grandchildren, Jeffrey, Ryan and Adam; and many extended family and friends.
Memorial services will be held on Friday, April 22, 2016 at St. Pius X Church at 10:30a.m.  Burial will be private.  Memorial tributes may be directed to the Alzheimer’s Association or Hospice of Northwest Ohio.   The family would like to express a special thank you to the staff at Parkcliff, especially Heather and Shanae, for all of their compassionate care and support.

Dear Red, Mike, Tony, Annie, and Gary my heart felt sympathy.
I have such wonderful memories of you all especially Grace coming over to my back porch and everyday sharing a root beer float with Ed and I.Especially spending summer days with Gary and Annie. I am very sorry for your loss.
