With broken hearts and tears in our eyes. We can say this is the most horrible thing to happen in our lives.  Jack Travis Swert went to be with our God at sunrise on September 5, 2019. Ironically, he was born at sunrise on May 20, 2006.
    He was a magical being in our lives. He loved to play jokes and pranks on people. He was so many things, bright, intelligent, enthusiastic and funny. Always willing to help and be a gentleman. And he was our road dawg. He loved to be on the go and traveling.  These are just a few ways to describe him.  A few of his favorite things in this world were, BACON, bacon pizza, chocolate milk and Fortnite.
    Jack touched so many lives in his short life. From family and friends to doctors and nurses.  Jack was sick most of his life, but always made the best of each day. He was a happy person no matter what life threw at him. Jack was wise beyond his years he was prepared and ready for Heaven. We will miss him every single day for the rest of our lives.
    Jack is survived by parents, Steven Swert and Nicole Rising; siblings, Isaiah, Kaiden and Olvia Swert; grandparents, Clifford and Michelle Rising, Lori Forman; great grandparents, his GG Sharon Morrin, great grandmother, Nancy Swert and Jane Forman; and by many uncles, aunts, cousins and friends.
    Jack went to school at Glendale-Fielbach.  He was a member at Cedar Creek Church south campus.  He loved going to the Fusion Youth Group and loved its leaders.
    Family and friends may visit at the W.K. Sujkowski & Son Funeral Home, 3838 Airport Hwy.  on Tuesday from 3-8 p.m. Funeral Services will be held on Wednesday, September 11, 2019 at 11 a.m. in the funeral home.  Interment will follow at Ottawa Hill Memorial Park Cemetery.  
  The family would like to acknowledge the doctors and nurses at Toledo Children’s Hospital. Especially Dr. Mark G. Watkins.

I'm sorry for your loss Jack was a great kid at camp and outside he put a smile on all of our faces. At camp he would love to tell jokes and he made all of us laugh. Camp won't be the same without Jack, he will be missed. Praying for your family.

To the family of Jack,
Our family is incredibly saddened for the loss of your son Jack. My boys got to know him at DYS camp this summer, and my older son Jon was one of his cabin leaders at Cedar Creek camp this summer. Even though their time together was brief, they enjoyed getting to know him. May God hold you during this painful time.

To the parents of Jack,
He was such a bright young lively individual, he battled through so much in his life while enduring so much will for life. His art was so fantastic, he had such an eye for good art, practiced so much he could've been his own inspirational artist. It's saddening to see such a young spirit be called home, to know he's no longer suffering in pain/sickness, dancing an living the dream with the heavenly Father. I pray that the giref your family and all the people who knew Jack will not only get easier, but find a way to know he's living through you each an everyday you keep fighting forward. He was remarkable for knowing at a young age what could possibly come from being sick and even then he wanted to live everyday as if it was his last with the biggest smile on his face. My condolences goes out to your family at this time.

I am so sorry and heartbroken for your family. Jack was an amazing, kind, funny, sweet, and hardworking student. He was smart too! I always remember how quickly he learned his letters and sounds. He literally learned all the letter sounds in a couple of weeks. Every time he saw me in the hall, he always said hi and gave me a hug. Jack was a wonderful kid with a big heart. I will be praying for you all during this difficult time.

Fly high jack we all will miss you,I really didn’t get to know you very well but the things I knew about you were kind and so sweet. Did it really have to come to a end in a snap,See you soon

Jack was one of my friends from glendale feilbach last year. He was funny, kind and cool, he loved to play fortnite. Jack was one of the cool people I've known. It's sad to see him pass away so young. Rest In Peace my dude.
